    Can you get under a cottage on stumps to look, or is that a crawl too far?
    Usually yes, and it is one of the advantages of the older housing here. A subfloor gives access to pipework that would be buried in concrete on a newer build, so a leak can often be seen rather than deduced. The failures are less common on those homes, but they are not impossible.
    Does a leak under the slab put the footings at risk?
    It is worth taking seriously on reactive clay. Water going into ground that already swells and shrinks with the seasons changes how that ground behaves under a footing, and persistent wetting in one area is not something you want continuing for years. It is a reason to act rather than a reason to panic.
    Once it is found, how big a hole are we talking about?
    As small as the repair allows, which is the entire point of locating it first. When the position is known to within a small area, the opening is sized to reach the pipe and no more. The alternative, cutting where the damp appears and working outward, is how people end up with a floor full of patches.
